PUBLIC FUND CHECKLIST

The following checklist is provided to ensure that all requirements have been met before an Application for Public Funds is filed:

􀂅 File Statement of Intent to Seek Public Funds (Form CC-4)

􀂅 File Affidavit to Voluntarily Agree with Campaign Expenditure Limits (Form CC-2)

􀂅 File Nomination Papers

􀂅 Qualified Opponent filed Nomination Papers

􀂅 All Reports have been filed and there are no Outstanding Penalties

􀂅 Collect Minimum Amount of Qualifying Campaign Contributions established for respective office

􀂅 Electronically file the Statement of Qualifying Campaign Contributions (Form CC-6(A)) on the Candidate Filing System and file a copy with the manually filed Application for Public Funds (Form CC-6)
