The following checklist is provided to ensure that all requirements have been met before an Application for Public Funds is filed: File Statement of Intent to Seek Public Funds (Form CC-4) File Affidavit to Voluntarily Agree with Campaign Expenditure Limits (Form CC-2) File Nomination Papers Qualified Opponent filed Nomination Papers All Reports have been filed and there are no Outstanding Penalties Collect Minimum Amount of Qualifying Campaign Contributions established for respective office Electronically file the Statement of Qualifying Campaign Contributions (Form CC-6(A)) on the Candidate Filing System and file a copy with the manually filed Application for Public Funds (Form CC-6) The three components to public funding are: minimum qualifying campaign contribution amounts, maximum public fund amounts, and an expenditure limit.
