Name changes, adding and deleting a class of insurance, redomestications, mergers, etc.
Licensed insurance companies submitting corporate amendments must use the Uniform Corporate Amendment Application Forms located at the Uniform Certificate of Authority Application (“UCAA”) website.

Corporate amendments include name changes, adding or deleting a class of
insurance, redomestications, mergers of foreign insurers, etc. If you
