The Executive Office on Aging’s Kokua Mau project announces
its collaboration with Health Directive Partners, LLC.
Healthcare Directive Partners, LLC is pleased to announce the
launching of its new web site, WWW.MyHealthDirective.com. Available free of charge to all
Hawaii residents, the site serves as a secure means for residents to
store their Advance Healthcare Directives and make them available to
treating physicians and hospitals.
It is also a web site intended to foster communication about
end-of-life decisions, including how to talk to your family and doctor
about these important decisions.
The site offers additional informative pages, in an easy to
understand format, and includes a resource directory, and an Ask a
Doctor, and an Ask an Attorney sections.
MyHealthDirective.com is owned by Integrated Services Inc. a
subsidiary of the Hawaii Medical Service Association, and is licensed
exclusively in the State of Hawaii to Healthcare Directive Partners,
LLC.
Healthcare Directive Partners, LLC is working with Kokua Mau to
promote the usage of the web site, and educate the community about
Advance Healthcare Directives and other end-of-life decisions. Kokua
Mau is a statewide coalition of organizations, community leaders, and
policy makers from the areas of health care, education and government
-- who are committed to improving how we in Hawaii are cared for at the
end of our lives. Coordinated through the Executive Office on Aging,
the lead partners are Hawaiian Islands Hospice Organization (composed
of the state's seven hospices), St. Francis International Center for
Healthcare Ethics, and the University of Hawaii Center on Aging.
Kokua Mau's mission is "to weave a lei of community support so that
Hawaii's people may die in the place of their choice, free of pain and
suffering, and treated according to their beliefs and values."
