VFC/TEEN VAX VACCINE ORDER FAX LINE: (808)

- All VFC and Teen Vax orders must be placed via fax. If you do not have a
fax machine please call the VFC Program at 586-8300 (neighbor islands call
1-800-933-4832) so that other arrangements for ordering can be made.
- To avoid delivery errors and vaccine wastage, be sure to enter your current
street address, telephone number, contact person, times you may receive delivery
(remember lunch periods and half days), and any vacations or holidays that
may be occurring within the two weeks following the placement of your order.
- If you have any questions regarding the ordering of VFC/Teen Vax vaccine
please contact the VFC office at 586-8300 (neighbor islands call 1-800-933-4832).
