If your method of filing your General Excise, Withholding, Transient Accommodations, or Rental Motor Vehicle and Tour Vehicle Surcharge tax return does not require a tax booklet to be mailed to you (e.g., you are filing through our E-Filing website or downloading your own copy of the forms), you may submit a request to the Department of Taxation to stop receiving a tax booklet. Please fill out and submit the request form below.

Note that this form cannot be used to cancel your account. To cancel your account, you must mail the GEW-TA-RV-1 Notification of Cancellation form to the appropriate tax district office. A copy of the form can be downloaded here. Do not fill out the form below if you will be canceling your account.
